    Proof of address<\/strong><\/td>\nUtility bill, credit card statement or any other official document with your name and address, that can prove your residency.<\/td>\n<\/tr>\n
    Driver's License<\/strong><\/td>\nCDL (Commercial Driver License) or CLP (Commercial Learner's Permit) if relevant, or a usual driver's license if you apply to drive a car.<\/td>\n<\/tr>\n
    Background check<\/strong><\/td>\nAny background check documents that are required across the US.<\/td>\n<\/tr>\n
    Motor Vehicle Record(s) (MVR)<\/strong><\/td>\nYour motor vehicle records. For CLP\/CDL drivers, your proof of medical certification must be shown on the MVR.<\/td>\n<\/tr>\n
    Road test\/certificate<\/strong><\/td>\nRoad test\/certificate less than 3 years old, or photocopy of a CDL accepted in lieu of road test.<\/td>\n<\/tr>\n
    Drug and alcohol records<\/strong><\/td>\nIf relevant<\/td>\n<\/tr>\n
